PhysioLogix Physiotherapy + Pilates (“we”, “us”, “our”) operates a physiotherapy and pilates clinic at 6/567 Newcastle St, West Perth WA 6005. We use your information to provide physiotherapy services, manage appointments and billing, process health fund, WorkCover, ICWA, NDIS and DVA claims, communicate with you and your other treating practitioners, and meet our legal and professional obligations.

We may disclose your information to your referring GP and other treating practitioners involved in your care, your health fund or relevant insurer for claiming, and third parties where required or authorised by law. We do not sell your personal information.
